New Patient Registration
The doctors welcome new patients who live within our practice area.
New patients wishing to register with the practice will be asked to fill in the required form, and if over the age of 5, will be asked to attend a New Patient Check. This will be with the Healthcare Assistant who will take blood pressure, height & weight measurements and ask you to provide a urine sample. Details of your medical history and current treatment will also be recorded.
If you have your NHS Medical Card, please bring it to your New Patient Check. This information will help the practice provide the correct medical attention.
You should have your New Patient Check before requesting services from the practice.
If you need regular medication, it would be useful to bring your repeat prescription form with you as it can take several weeks for all your records to be transferred.
Please also note that you will be registered with the practice, rather than with an individual doctor, but you can ask to see a specific doctor when you make an appointment.
